Preparing For Your First Visit
Thank you for choosing TuDor Physical Therapy! We want to ensure that your first visit is as smooth as possible and that all of your questions are answered. To help us better serve you, please arrive 15 minutes prior to first evaluation and bring the following to your appointment:
- Photo ID
- Current insurance card(s)
- Referral from your physician
- (Or) Non physician referral form (download)
- Any questions you might have for your therapist

Prior to your first visit, please review and complete the Patient Information Packet and any other appropriate forms, and bring them with you to your first appointment.
